LE Training Outreaches are when Literature Evangelist (Independent Book Distributors) come together to canvass, in support of a local church outreach.  LE Outreaches are an important time of training and the opportunity for church members to experience front-line mission work here in Australia. Presenting our books of truth to the people is the very work the Lord would have us do at this time in world history. By joining this team, you are agreeing to represent Jesus and the Seventh-day Adventist church in your positive attitude, and respectful conduct.

The information contained in this form is true and accurate to the best of my knowledge. I wish to attend and participate. I acknowledge that there are risks (injury, loss or damage) involved and choose to participate on this understanding knowing that all reasonable care will be taken to avoid such.  I acknowledge that the LE Outreach is not responsible for the loss or theft of my personal property. Lost property will only be held for 21 days after this event ends.

In the event of accident or illness, I authorise the Directors to consent; where it is impractical to communicate with me, to receive my or my Next of Kin consent; for me to receive any x-ray examination, anesthetic, medical, surgical or hospital treatment as may be deemed necessary by a licensed physician and/or surgeon.

I acknowledge that photos may be taken and consent for them to be used for marketing purposes.
